STARS Application Form

The Elizabeth Education Foundation is pleased to announce the 27th Annual STARS Celebration.

Please note the Elizabeth Education Foundation will only accept two nominations per faculty member.

Nominees for the STARS award should demonstrate some of the following characteristics:

Model responsible behavior
Accountable for their actions
Spends time involved with clubs or organizations at school and/or actively supports the community
Committed to learning, motivated to do well in school, actively engaged in learning, and cares about his/her school and community
High value on caring and helping other people and accepts personal responsibility
Makes good choices, has empathy, sensitivity, friendship skills, and resists negative peer pressures

The nominees, their two guests, and the nominator will be invited to the STARS Celebration on April 12th, 2025.

Recipients can only receive this honor once during their school career. To prevent duplication, a list of past years’ recipients is available. Should you need to refer to this list, please review these students here.

All nomination forms must be submitted by February 21st, 2025.

Breakdown of nominees per school:

Legacy Academy: 6 (2 for 5th grade, 1 per grade 6, 7, 8 plus 1)

Running Creek Elementary: 3 (5th grade only)

Singing Hills Elementary: 3 (5th grade only)

Elizabeth Middle School: 14 (4 per grade, plus 2)

Elizabeth High School: 24 (5 per grade, plus 4 )

Please write a brief explanation as to why you feel the student you have selected is deserving of this award. This statement will be presented to the student on a personalized certificate and read by our emcee. Please limit the number of words to at least 25 and no more than 75. It will also be used as a guideline for the master of ceremonies when introducing the nominee during the STARS celebration.
